Liposuction
Overview
Liposuction is designed to remove fat deposits from specific areas that have not responded well to diet and exercise. The procedure is most commonly recommended for men or women who are of normal body weight but who have fat deposits in certain areas that make these areas appear disproportionately large. The procedure can remove stubborn areas of unwanted fat from the body, face, or neck to bring these areas into proportion with the rest of the body.
The "Tumescent" liposuction technique is a two-step procedure involving the infusion of a tumescent fluid containing a long-acting local anesthetic (to lessen pain after surgery) and a vasoconstrictor (to reduce bleeding and bruising) into the area of unwanted fat followed by removal of the fat cells by suction. One or more small incisions, approximately quarter-inch, are used for each area and, wherever possible, are hidden in a natural crease to make them inconspicuous.

For those who do not have insurance, Dr. Phan charges approximately $120 for surgical clearance, including EKG & bloodwork.
The Procedure
The procedure takes approximately forty-five minutes to ninety minutes per area. Some areas, such as the hips and outer thighs may be done under local anesthesia. Other areas, including the abdomen, inner thighs, back and arms must be done under general anesthesia. We use only board certified anesthesiologists to care for our patients. After surgery, you will recover in the recovery area for approximately one hour, until you are alert enough to go home. You will be wearing a supportive garment that you will continue to wear for several weeks after surgery. The sutures are absorbable and should fall off within two weeks.
The Day of Surgery
While you're making preparations, be sure to arrange for someone to drive you home and be with you overnight. Your surgery will be performed in our outpatient surgery center at 11190 Warner Avenue, Suite 400; Fountain Valley, CA. If your procedure is done under local anesthesia, you may eat and drink as normal. If your procedure is being done under general anesthesia, you will not be allowed to have anything to eat after 9pm the evening before surgery or drink after midnight the night before surgery. You should shower the day of surgery but do not use any lotions or perfumed products on your body. Wear loose, comfortable clothing. Arrive two hours prior to surgery for general anesthesia or one hour prior to surgery for local anesthesia. You will need someone to bring you on the day of surgery and take you home and stay with you that night.
After Surgery
For the first few days, you will need help getting to the restroom. You will begin taking pain medication and antibiotics after surgery. Each time you take your pain medication, you should have a snack as the medication can be irritating to your stomach. For the first 24 to 48 hours you will have a sensitive stomach and you should eat mild foods such as you would normally eat if you had the flu (soups, crackers, etc.). Dr. Nguyen also strongly suggests Gatorade to replenish your fluid levels.
You will be able to return to most of your normal (light) activities within a couple of days. You will be able to shower once you feel comfortable being up for extended periods of time without lightheadedness, usually on the third day. You will probably need help taking off your garment and putting it back on. Patients with an office job are often able to return to work within 3-7 days. More strenuous jobs may require work restrictions for eight weeks, which you may request a certifying letter from the receptionist.
During the first week, swelling will reach its peak and will slowly subside. You may return to most of your normal ("light") activities within one week including non-strenuous work. Over the next several weeks, the swelling will continue to subside, bruising will fade, and normal sensation will slowly return. Although you will notice a difference in the contour of the area shortly after surgery, the improvement will become even more apparent as the swelling and fluid retention subsides over several weeks to months. You will notice that your clothing fits more comfortably and you will enjoy a new, slimmer appearance. Your new contours will be permanent providing your weight is stable. In addition most patients notice that changes in weight do not result in accumulation in these problem areas as it did before.
After two weeks, and once all incisions are well healed (no open areas or scabs), you can begin using Scar Guard® scar treatment on the incisions. Scar Guard® is available at the pharmacy in our building.
Your Results
Most patients are very pleased with their new shape. You will notice that your clothing fits more comfortably and you will enjoy a new, slimmer appearance. Tummy Tuck
Overview
If you are frustrated by a protruding abdomen, excess abdominal fat, and loose, sagging abdominal skin which do not respond well to diet and exercise, abdominoplasty ("tummy tuck") may be a good option for you. A protruding abdomen often results from weak abdominal muscles, weight gain, or pregnancy. Diet and exercise may have limited effect on this condition because of irreversible stretching of the skin and underlying tissues. Abdominoplasty flattens a protruding stomach by tightening muscles and removing excess fat and skin. The result is a more pronounced waistline and a flat, firm abdominal area. You will discuss with your surgeon the placement of the incision, which can usually be concealed beneath most undergarments or swimsuits.

Preparing For Surgery
On the day of your consultation, you will receive instructions on how to optimize your health before surgery. This will include what medications to stop and what testing will be required before surgery. If you are 45 years of age or older, or if you have any health problems, you will need to see your primary care physician to have bloodwork, an EKG, and obtain surgical clearance stating that there are no counterindications for you having surgery. You should not take aspirin containing products or smoke for two weeks before surgery. If you have health problems or a history of health problems, you may require additional testing or clearance.
The Procedure
The procedure takes approximately three hours. The procedure involves an incision just above the pubic area extending outward toward the hips with the length of the incision dependent upon the amount of skin to be removed. The surgeon will work through this incision to tighten the underlying tissues and remove excess fat from the abdominal area. Excess skin will then be removed. If there is a large amount of excess skin to be removed, a second incision will be made around the navel. The excess skin from the navel to the incision at the pubic area will be removed. The skin will then be pulled down to the incision at the pubic area. The navel will remain in place but may change shape slightly. If present, some or all abdominal stretch marks may also be removed at this time. C-section scars can usually be incorporated in the incision. You will be wearing a supportive abdominal garment, will have surgical tapes over the incision, and will have drainage tubes to help prevent fluid accumulation in the abdominal area. After surgery, you will recover in the recovery room until you are alert enough to go home.

You should rest with your upper body elevated (approximately 45 degree angle) so that you can get in and out of bed more easily. One or two pillows should be placed under your knees for comfort.
You will begin taking pain medication and antibiotics after surgery. Each time you take your pain medication, you should have a snack as the medication can be irritating to your stomach. For the first 24 to 48 hours you will have a sensitive stomach and you should eat mild foods such as you would normally eat if you had the flu (soups, crackers, etc.). The drainage tubes are usually removed within the first week, during your follow-up appointment. You may shower the day after the drains are removed. The tapes over your incision will protect the incision from the soap and water so you may shower as normal and pat the tapes dry. The surgical tape will begin to fall off during the second week and can be removed at the end of the second week. Patients with an office job are often able to return to work after about two weeks. More strenuous jobs may require work restrictions for eight weeks, which you may request a certifying letter from the receptionist.
Over several weeks, the swelling will continue to subside, bruising will fade, and normal sensation will return. Although you will notice a difference in the contour of the area shortly after surgery, the improvement will become even more apparent as the swelling and fluid retention subsides over several weeks to months. You will notice that your clothing fits more comfortably and you will enjoy a new, slimmer appearance. Most patients are very happy with their firmer, flatter abdomen and their renewed confidence in the way they look in clothes. Your new contours will be permanent providing your weight is stable. You will continue to wear a supportive garment for six to eight weeks. During this time, you will slowly return to your normal activities. Over the first three to six months, the last of the swelling, along with feelings of numbness or tightness, will disappear. Your scar will begin to fade and will continue to improve for the first year.

Breast Augmentation
Overview
If you are like many women who desire larger, fuller, shapelier breasts, breast augmentation may be a good choice for you.
Breast augmentation is a common procedure used to enhance small breasts or correct breast volume reduction due to pregnancy, weight loss, or the natural aging process. The procedure results in fuller, more beautifully shaped breasts.
Options in Breast Augmentation
OUTER SHELL: MATERIAL & SHAPE
The outer shell of the breast implants is made of a solid silicone polymer. This substance is used for implants placed anywhere in the body because it is an inert substance with no possibility of allergic reaction or rejection. Based on individual factors, the outer shell may be smooth or textured. Smooth implants are usually preferred because of the reduced amount of wrinkling and more natural feel and appearance. Textured implants may be used for patients who have had implants before and have developed capsular contracture as it is believed by some surgeons that the use of textured implants may prevent the reoccurrence of capsular contracture.
Breast implants are round or anatomical (“teardrop-shaped”). Round implants are the most common and are designed to compliment the anatomy of most women. For women who are very tall and/or have a longer-than-average torso, anatomical shaped implants may be used. Anatomical implants have a greater vertical component and give some height to the breast when needed to proportionately fill the height of the chest.
One of the most important decisions to make is the implant size. During your consultation, Dr. Nguyen will discuss your desired results and will recommend a size that will be appropriate for your height, weight, body frame, and desired fullness. To help you visualize the results you can expect, your patient coordinator will try the sizes on with you. Your feedback from the sizing will be relayed to Dr. Nguyen and adjustments may be made to his recommendation.
There are also different profiles of implants, referring to the amount of projection given a certain implant volume. Determining which profile is appropriate depends on a combination of desired volume and chest width. A moderate profile implant is appropriate for most patients (ie. women who require a moderate enhancement). If the moderate implant diameter is significantly larger than the width of your breast, a high profile implant may be chosen – giving more projection with a smaller diameter. If your breast width is between the diameters of the moderate and high profile implants, you will be asked to decide if you prefer more upper pole fullness (more volume at the top of the breast) or more fullness to the side of the breast. For more fullness at the top of the breast a high profile implant will be chosen whereas for more fullness at the side of the breast, a moderate profile implant will be chosen.
Saline vs. Silicone
For most patients with a first time breast augmentation, the implant will be saline-filled. Saline breast implants are filled with sterile saline solution, a fluid of similar chemical composition to the fluid in your body. The advantage of saline breast implants is that the fluid is completely harmless and in case of a leak in the implant, the fluid would be easily absorbed by the body. Most patients are very happy with saline implants as they look beautiful and are very safe. Patients who are very thin or who have little breast tissue may feel the implant or notice some wrinkling of the implant on the lateral edge (closest to the underarm) or under the breast at the inframammary fold.
There may be some patients who would prefer silicone breast implants. The advantage of silicone breast implants is that they feel more like real breast tissue and have less wrinkling than saline implants. Women with very little breast tissue and/or very thin skin would benefit most from the reduction of wrinkling and the more natural feel of silicone breast implants. The disadvantage of silicone filled breast implants is that although there is promise for approval by the FDA in the near future, they are currently limited to patients who have had problems with saline implants (eg. capsular contracture) or who are undergoing reconstructive breast surgery (including breast lift and post-mastectomy reconstruction). There have been great advances in silicone implants since they were originally taken off the market. The current generation of silicone gel implants is filled with a cross-linked polymer of silicone making it a gel rather than a silicone liquid (as the previous generation contained). Inamed also uses a patented Intrabarrier Shield® to prevent migration of the silicone gel across the shell. We feel that these silicone implants, which are currently being widely used for qualifying patients, are relatively safe and will eventually be brought to the general market.
Implant Placement: Over vs. Under The Muscle
Implants may be placed directly under the breast tissue but it is more common to place the implant under the pectoral muscle. There are several important reasons to place the implant under the muscle. First, the muscle covers the top of the implant and provides a smooth transition, giving a natural look to the breast, even in revealing clothing. The muscle also provides additional support to the implant, giving longer lasting results. For mammograms, radiologists prefer the implant to be placed under the muscle so that the breast tissue can be isolated more easily for accuracy of the imaging. Finally, placing the implant under the muscle reduces the risk of capsular contracture (the hardening of scar tissue around the implant) by as much as two thirds. The rate of capsular contracture for implants placed over the muscle is approximately 10% whereas rate of capsular contracture under the muscle is approximately 3-5%.
Incision Area
The incision is approximately 2cm in length (less than one inch) and is usually placed either at the bottom edge of the areola (the pigmented area around the nipple) or along the inframammary fold (the natural crease under the breast). Scarring with the periareolar incision is minimal due to the color contrast provided by the areola and minimized tension on the incision at that site. The incision is thin and small under the inframammary fold and will hide most of the time, but some bras and bikinis may pull up on the breast revealing the incision at that location.
Alternate methods of implant placement include endoscopic techniques with incisions under the armpit (transaxillary) or through the belly button (transumbilical / TUBA). The TUBA technique does not allow for placement under the muscle and these techniques may also carry additional risks including internal scarring and malplacement of the implant and are therefore not preferred by most plastic surgeons.

Your first follow-up visit will be at one week after surgery. You will be taught to massage your breasts at this time which will help the muscle to relax and the implant to settle into place. You will be reminded that the tapes over the incision may be removed after the second week.
During the first six weeks you will wear a supportive bra with no underwire. You may continue to wear the one we provide or purchase an alternative such as a soft sports bra with hooks or a zipper in the front. During this recovery period, you will also reduce your activities by eliminating strenuous aerobic exercise, limit lifting (to no more than 5 lbs), and reduce the amount of work you do with your arms. More strenuous activities may be reintroduced after six weeks. You should ask your doctor about specific activities.
Commonly Asked Questions
Is it likely that I will lose nipple sensation after breast augmentation?
No. It is extremely unlikely that normal nipple sensation will be disrupted in any way by breast augmentation. There will be some temporary changes in breast sensation which will resolve in approximately 8 weeks.
Are breast implants dangerous to my baby if I become pregnant or decide to breast feed?
No. Breast implants do not harm or pose a risk to normal pregnancy in any way.
Can I breast feed after breast augmentation?
Yes. Most women who have breast augmentation should be able to breast feed. However, there is some chance that the milk supply may be diminished from the pressure of the implant.
What will happen to my breasts if I become pregnant?
Pregnancy and breast feeding may result in some sagging of the breast. Because the implant maintains its own integrity and does not lose volume with breast feeding, women with implants will experience less changes than women without breast implants.

Breast Lift
Overview
If you wish your breasts were firmer, higher, and more youthful, a breast lift may be the answer. One of the most common complaints of women is sagging or loose breasts which may result from pregnancy, weight loss, or the natural aging process. Mastopexy, commonly called breast lift, is the technique used to elevate and reshape the breast.
There are three types of breast lifts depending on the degree of ptosis, or sagging, of the breast. The first type, called a periareolar breast lift, is for minimal ptosis and involves an incision around the areola only. The procedure involves removing some of the skin above the areola and elevating the areola to a higher position. The second type of breast lift, sometimes called a “lollypop” lift, involves an incision around the areola and down the breast. This gives more lift to the breast and elevates the nipple more than the periareolar lift. The third type of lift, sometimes called an anchor lift or inverted “T” lift involves an incision around the areola, down the breast and along the inframammary fold, the natural crease under the breast. This type of lift gives the most lift to the breast and gives the most elevation of the areola. Nipple sensation and the ability to breast-feed are not usually compromised by this procedure.

The Procedure

Breast Reduction
Overview
If you feel that your breasts are too large and out of proportion with the rest of your body, a breast reduction may be the answer. Women with unusually large breasts may feel physical discomfort and a breast reduction is often performed for physical relief rather than simply cosmetic improvement. The weight of large breasts may cause back and neck pain and skin irritation. Bra straps may leave indentations in their shoulders. Emotionally, women may feel extremely self-conscious about their bodies because of the size of their breasts. Breast reduction surgery produces breasts that are smaller, better-shaped, and more proportionate with the rest of your body.

You may also have drainage tubes to help prevent fluid accumulation in the abdominal area.

Your results will be long lasting as long as you do not become pregnant or gain a significant amount of weight. It does take most women several months to get used to their new shape; but in the end, most women are very pleased with their new body image.

Diligent use of cold compresses during the first three to five days will significantly decrease your swelling and bruising and decrease healing time. You should also rest your eyes and not watch too much television or read for long periods of time because the eyes can also become swollen. Your healing incision lines may itch a little. Avoid scratching them.

Bandages and some of the sutures will be removed at one week. Post surgical swelling may temporarily cause the look of unevenness, skin changes, hard spots, and ridges. Do not panic, this will go away. After the first week much of the remaining discoloration can be concealed with a cream-based cosmetic. Do not apply cosmetics to any incision line before 48 hours after suture removal. Hair can be washed after your one-week follow up.
The remainder of the sutures will be removed at two weeks and you may resume many of your normal activities, including light work. After two weeks, and once all incisions are well healed (no open areas or scabs), you can begin using Scar Guard® scar treatment on the incisions. Scar Guard® is available at the pharmacy in our building.
Most of the bruising and much of the swelling will have disappeared by the second week. The remainder of the swelling will continue to subside over the next several weeks. Pink scars will eventually fade to an unnoticeable white color but this may take up to a year. There will be some numbness of the face, which will continue to diminish over several months. You should wait eight weeks for chemical hair treatments, including hair color. After eight weeks you may also resume your normal exercise routine.
Your Results
You can expect to enjoy a revitalized and more youthful appearance. Most people will not be able to tell that you had surgery but they will feel that you look happier, healthier, and more well-rested. Although you will continue the normal aging process, you will do so beginning with your new, more youthful appearance. Frozen peas in a ziplock bag wrapped in a thin towel work well to contour to your lower face and provide adequate cooling. You may experience some numbness so remember not to put ice or anything hot directly onto skin because you may have decreased awareness of temperature damage. Diligent use of cold compresses during the first three to five days will significantly decrease your swelling and bruising and decrease healing time. Your healing incision lines may itch a little. Avoid scratching them.
You will begin taking pain medication and antibiotics after surgery. Each time you take your pain medication, you should have a snack as the medication can be irritating to your stomach. You should eat soft foods and avoid talking to reduce swelling. For the first 24 to 48 hours you will have a sensitive stomach and you should eat mild foods such as you would normally eat if you had the flu (soups, yogurt, etc.). Dr. Nguyen also strongly suggests Gatorade to replenish your fluid levels.
Bandages and some of the sutures will be removed at one week. Post surgical swelling may temporarily cause the look of unevenness, skin changes, hard spots, and ridges. Do not panic, this will go away. After the first week much of the remaining discoloration can be concealed with a cream-based cosmetic. Do not apply cosmetics to any incision line before 48 hours after suture removal. Hair can be washed after your one-week follow up.
The remainder of the sutures will be removed at two weeks and you may resume many of your normal activities, including light work. After two weeks, and once all incisions are well healed (no open areas or scabs), you can begin using Scar Guard® scar treatment on the incisions. Scar Guard® is available at the pharmacy in our building.
Most of the bruising and much of the swelling will have disappeared by the second week. The remainder of the swelling will continue to subside over the next several weeks. Pink scars will eventually fade to an unnoticeable white color but this may take up to a year. There will be some numbness of the lower face and neck, which will continue to diminish over several months. You should wait eight weeks for chemical hair treatments, including hair color. After eight weeks you may also resume your normal exercise routine.

You can expect to enjoy a revitalized and more youthful appearance. Most people will not be able to tell that you had surgery but they will feel that you look happier, healthier, and more well-rested. Although you will continue the normal aging process, you will do so beginning with your new, more youthful appearance. Eyelid Surgery
Overview
Your eyes are one of the first places that will show signs of aging. Exposure to the sun, genetic predisposition, and the natural aging process may cause wrinkles and puffiness around the eyes. As you age, skin of the upper lid may begin to droop, lines may form around the bottom of your eyes, and bags may begin to form. Eyelid surgery can help by removing excess fat and tightening the skin of the eyelids to give a brighter, more rested appearance. If you feel that your eyes are making you look tired or older than you feel, eyelid surgery may be a good option for you.
The Consultation

Preparing For Surgery
On the day of your consultation, you will receive instructions on how to optimize your health before surgery. This will include what medications to stop and what testing will be required before surgery. If you will be undergoing general anesthesia and are 45 years of age or older -or- have any health problems, you will need to see your primary care physician to have bloodwork, an EKG, and obtain surgical clearance stating that there are no counterindications for you having surgery. You should not take aspirin containing products or smoke for two weeks before surgery. If you have health problems or a history of health problems, you may require additional testing or clearance.
The Procedure
Blepharoplasty (eyelid surgery) involves the removal of excess skin and fat on the upper and/or lower lids, giving the eyes a more youthful and rested look. For the upper eyelid, the incision is made in the natural fold of the upper lid, from the inside corner to the outside corner, often extending just beyond the eye to the "crow's feet." The incision is used to allow removal of excess fat and skin. When healed, the incision is well concealed within the natural contour of the lid. For lower blepharoplasty, the incision is most often made just below the lashes of the lower lid. Again, the incision is used to allow removal of excess fat and skin. When healed, the incision is usually imperceptible. For individuals who need only removal of fat but not skin, the incision may be made inside the eyelid. This technique eliminates the need for an external incision but limits the results to fat reduction only. If environmental damage has caused fine lines around your eyes, you may consider adding a chemical peel to the procedure to help eliminate these lines and improve your overall results.
The Day of Surgery
Blepharoplasty is an outpatient procedure, which may be performed under local anesthesia or general anesthesia. It may be performed alone in conjunction with other facial rejuvenation procedures such as facelift. The surgery takes approximately forty-five minutes for each the upper and lower lids. After surgery, you will recover in the recovery room for a short time and then you will be released to go home.
While you're making preparations, be sure to arrange for someone to drive you home and be with you overnight. Your surgery will be performed in our outpatient surgery center at 11190 Warner Avenue, Suite 400; Fountain Valley, CA. If performed under local anesthesia, you may eat and drink like normal. If performed under general anesthesia, you will not be allowed to have anything to eat after 9pm the evening before surgery or drink after midnight the night before surgery. You should have clean, dry hair. Wear no makeup or moisturizers on your face. You should not bring jewelry, contact lenses, or valuables on the day of surgery. Wear loose, comfortable clothing with a shirt that buttons or zips in the front (we don't want to put anything on over your head when you get out of surgery). Arrive one hour prior to surgery for local anesthesia or two hours prior to surgery for general anesthesia.
After Surgery
You will be given prescriptions for pain medication and antibiotics. There is usually very little pain with the eyelid surgery and most patients only take the pain medication for the first day or two. You should begin taking your antibiotics when you get home and should finish the entire prescription.
For the two to three days, you will rest much of the time with your head elevated (approximately 45 degree angle) using a cold compress on your eyes to reduce swelling and bruising. Upper eyelids will have sutures to be removed at approximately one week and there will be a small tape over the sutures to protect them during this time. Besides some bruising and swelling, some patients may experience temporary blurred vision, tightness, or discomfort. You will be able to return to most of your normal activities within one week. Most patients return to work one or two weeks after the procedure. Most of the swelling will subside within a few weeks and you will be able to see your final results.

Botox
WHAT IS BOTOX®?
Botox® is a purified toxin approved by the FDA in the late 1980s (Food and Drug Administration) for use to treat certain optic muscle disorders. When using Botox® for this purpose, some doctors noticed a reduction in the appearance of wrinkles around the eyes as well. Allergan used this information to begin clinical trials for the use of Botox® for the cosmetic reduction of wrinkles. In April 2002, Botox® gained FDA approval for the treatment of moderate-to-severe frown lines between the eyebrows - called glabellar lines. Botox is also used for treatment of wrinkles around the eyes and on the forehead. Physicians have now been using Botox® for over a decade to successfully treat severe lines around and above the eyes and it is currently one of the most common cosmetic procedure performed.
How does BOTOX® work?
Botox® contains a chemical that inhibits signals from the nerves to the muscles. Wrinkles occur in the skin when overactive underlying muscles tighten, especially when facial expression are repeated often and over time. To treat these wrinkles, small amounts of Botox® are injected with a fine needle into overactive muscles of the brow and around the eyes. The procedure is virtually painless, takes only a few minutes, and you can go right back to your normal activity. Over the next several days, the Botox® will begin to work by reducing the effectiveness of the nerve signals to the muscle which cause the muscle to react. The injected muscles will then begin to weaken in their contraction and the wrinkles will begin to relax and soften. Botox® is effective on forehead lines, crow's feet (lines around the eye) and frown lines.
HOW LONG DOES BOTOX® LAST?
The effects of Botox® will last four to six months. As muscle action gradually returns, wrinkles will begin to reappear. It is normally recommended that you schedule treatments approximately 4 months apart to maximize the benefits of the Botox® treatments and prevent reoccurrence of undesired wrinkles.
BOTOX® COUNTERINDICATIONS & SIDE EFFECTS
You should not be treated with Botox® if you are pregnant, if you think you might be pregnant, or if you are breastfeeding. As with any injection, Botox may result in bruising. You should avoid aspirin containing products and anti-inflammatory medications for two weeks prior to treatment in order to reduce bruising. Infrequently, patients may experience muscle weakness or drooping of the upper eyelid muscles. This side effect usually resolves itself within days up to four months.
THE COST OF BOTOX®
Botox® is usually sold by the unit or by the area. Botox® is usually sold for between $10-$12 per unit with the total cost of the procedure ranging between $125 to $600.
What is JUVÉDERM™?
JUVÉDERM™ injectable gel is a colorless hyaluronic acid gel that is injected into facial tissue to smooth wrinkles and folds, especially around the nose and mouth. Hyaluronic acid is a naturally occurring sugar found in the human body. The role of hyaluronic acid in the skin is to deliver nutrients, hydrate the skin by holding in water, and to act as a cushioning agent. Juvéderm™ injectable gel instantly restores your skin's volume and smooths away “jowl lines,” “smile lines,” and “parentheses.” It can also be used to add volume to lips and cheeks.
HOW LONG DOES THE TREATMENT TAKE AND HOW LONG DO IT LAST?
With Juvéderm™, the injection process takes only about 15-30 minutes. It may be done with local anesthesia to minimize discomfort. The results of Juvéderm™ are immediate and you may return to your normal activity within an hour or so. Results may last up to 1 year.
JUVÉDERM™ COUNTERINDICATIONS & SIDE EFFECTS
You should not be treated with Juvéderm™ if you are pregnant or think you may be pregnant, or if you have severe allergies, especially with history of anaphylaxis. Side effects were usually mild to moderate lasting 7 days or less and included temporary injection site reactions like redness, pain, firmness, swelling and bumps. Patients who are using substances that can prolong bleeding, such as aspirin or ibuprofen, as with any injection, may experience increased bruising or bleeding at the injection site.
THE COST OF JUVÉDERM™
Juvéderm™ costs approximately $500.00 per syringe. Most patient will use 1-2 syringes per treatment.

Rhinoplasty
Overview
An attractive nose is one that is in harmony with the rest of your facial features. If you feel that your nose is not in balance with the rest of your face, rhinoplasty (nose surgery) may be a good option for you. Rhinoplasty is a common procedure used to reshape the nose to bring it into balance with the rest of the face. Rhinoplasty can be used to straighten the nose, make it more narrow, remove a hump, or refine the overall shape of the nose. The result is a nose that looks natural and is in balance with your other facial features.
Rhinoplasty may be "closed rhinoplasty" involving an incision made inside the nose, or "open rhinoplasty" which involves another small incision is made across the columella, the strip of tissue which separates the nostrils. The surgeon uses these incisions to gain access to the bone and/or cartilage structures inside the nose. The bone or cartilage structures of the nose may be reduced, refined, or enhanced during rhinoplasty. Enhancement rhinoplasty involves the use of tissue from the nose, tissue from another body site, or a synthetic implant.

You should avoid blowing your nose and may clean your nose with a Q-Tip during this time. After several weeks, you will be able to return to most of your normal activities. Although most of the swelling will resolve within the first couple of weeks to months, it may take as long as nine to twelve months for your nose to completely heal and for all of the swelling to diminish. You may notice that the tip of your nose is hard and inflexible. This is normal and it will soften over the next several months.
Your Results
Rhinoplasty helps to refine your nose and improve the appearance of your nose. You can expect that your new nose to look natural and be in better balance with your other facial features. You may be surprised that many people who know you well will not be able to tell that you have had surgery. They may just feel that you look "better" and be unable to explain why.

Body Contouring
In recent years, Gastric Bypass surgery has grown significantly in popularity as a treatment for obesity. This procedure is designed to result in a significant weight loss (often over 100 lbs) over a relatively short period of time (usually 12-18 months). With such dramatic weight loss, the skin often does not retain the elastic qualities necessary to shrink down to your new underlying shape. This often leaves patients with excessive amounts of hanging skin -- resulting in disappointment with their appearance, not being able to find clothing that fit properly, as well as creating problems with exercise and daily hygiene.
Because of the increase of patients who are in need of body contouring after significant weight loss, this has become an important focus of our practice. It is important to know that going to a plastic surgeon who specializes in gastric bypass patients is critical because there are many special factors involved with significant weight loss patients. Because of the amount of skin to be removed and the loss of elasticity of this skin, these patients must be treated very differently from non-gastric bypass body contouring patients. What may work well on other patients simply does not provide adequate shaping and contouring for the gastric bypass patient.

Often, patients will have several areas they wish to address. During your consultation, you will be asked to prioritize your areas of concern. You will often be given a quote on several different procedures that can be done together. For instance it is fairly common to combine a breast lift (with or without implants) with an arm lift, or a breast lift with a tummy tuck or thigh lift, etc. Dr. Nguyen will work with you to determine the best plan for you to achieve your overall body contouring goals. Patients who have undergone gastric bypass surgery have similar areas of concern. The corrective procedures necessary to regain normal body contour are similar for most patients.
Face Lift / Neck Lift
Some patients find that loss of facial fat during weight loss leaves them with loose hanging skin on their face and neck, causing a prematurely aged appearance. A facelift or necklift may be indicated to regain a firm, youthful appearance.
Mastopexy (Breast Lift)
Many women find that as they lose weight, they lose volume in their breasts, often more than they desire. The reduced volume is especially apparent at the top of the breast, decreasing cleavage and causing bras and clothing to fit incorrectly. Many gastric bypass patients require a breast lift (mastopexy) to lift the breast and reposition the areola to a higher, more youthful position. An implant is often used in conjuction with a breast lift to add volume, especially to the top of the breast if a breast lift alone will not accomplish the patient’s desired volume and shape.
Breast Lift/Reduction for Men
Men may also develop excessive tissue on the chest with weight gain & weight loss. Breast lift procedures are performed for men as well, alone or with reduction, to reduce size of the breast and create a more masculine contour.
Brachioplasty (Arm Lift)

Panniculectomy / Tummy Tuck
The most common complaint of gastric bypass patients once they reach their goal weight is the loose hanging skin of the abdomen. The hanging skin of the abdomen, called the pannus or “apron” can cause a variety of problems for the patient. These include the inability to fit comfortably into clothing, difficulty in daily hygiene, yeast infections or cysts in the folds of skin, and lower back pain. The removal of this excess skin is acheived by panniculectomy, tummy tuck, or body lift procedures.
The term panniculectomy is used to describe the removal of the hanging, excess skin of the abdomen. This procedure does not include muscle tightening or liposuction. A panniculectomy may be covered by insurance if your plan allows for it and if you meet a series of standards set forth by the insurance regulators. A tummy tuck, on the other hand consists of three components: muscle tightening, liposculpture, and removal of excess skin. A tummy tuck is by definition a cosmetic procedure and is not covered by insurance.
Thigh Lift
Loose thighs may reduce the clothing styles you are able to wear and may cause problems such as rubbing and rashes on the inner thighs. A thigh lift is a surgical procedure to remove excess skin and fat from the thighs resulting in improved contour of the legs.
Lower Body Lift
A lower body lift is a common surgical procedure to remove excess skin and fat from the lower-middle body. This procedure tightens the abdomen and hips as well as lifting the buttocks and outer thighs. A lower body lift can markedly improve the contour of your abdomen, buttocks and outer thighs and will allow you to fit better in clothing and even wear clothing you have not been able to wear such as shorts or form fitting clothing.

Reduction of the breast may be covered if the weight of the breast is causing pain and/or other health issues. Documentation from your primary physician of the neck and/or upper back pain, rashes, yeast infection of the skin under the breast, or other physical symptoms may be required. A minimum amount of breast tissue (approximately 500g) must be removed to show medical necessity. Also, documentation may be necessary to show that conservative measures have been attempted and have failed to alleviate symptoms. These measures may include chiropractor visits, pain medications, topical ointments for skin infections and rashes, etc. Photos will be taken at the time of consult to show the severity of the drooping, skin rashes/lesions, or shoulder grooving.
Panniculectomy
Removal of excessive skin over the abdomen may be covered if the pannus (hanging skin) occludes the pubic area and is causing health problems. Documentation from your primary physician of back pain, rashes, yeast infection of the skin under the pannus, or other physical symptoms may be required. Also, documentation may be necessary to show that conservative measures have been attempted and have failed to alleviate symptoms. These measures may include chiropractor visits, pain medications, topical ointments for skin infections and rashes, etc. Photos will be taken at the time of consult to show the severity of the pannus and skin rashes/lesions if present. Panniculectomy is not a Tummy Tuck and does not include tightening of the muscle or liposuction.

The combined effects of aging, gravity, weight loss, and a loss of skin elasticity all contribute to a loss of tone in the arms. Brachioplasty, or arm lift, is a surgical procedure to remove excess skin and fat from the upper arm, reducing skin redundancy and improving the firmness of the skin in the upper arm area. As with all body contouring after major weight loss, it is best to wait until your weight has stabilized with no more than a 5 lb. loss per month over at least three months.
Brachioplasty is specifically tailored to meet the needs of each individual patient. The amount of excess skin and fat present in the upper arms will determine the extent of the incisions required. Rarely, minimal excess skin may be treated with an incision in the armpit alone. Most often, an incision from the armpit to the elbow is necessary to treat moderate laxity. More severe cases are rare and may require extension of the incision past the elbow onto the forearm. If a breast lift procedure is performed at the same time, the incision may be connected to reduce the excess skin on the sides of the breast below the armpit.

The Procedure
Brachioplasty surgery usually takes two to three hours. All sutures are placed beneath the skin and are absorbed by your body, leaving no sutures to be removed by the doctor. A sterile dressing is applied to the incisions, and a compression garment may be placed to support your arms during healing, decreasing postoperative swelling. After surgery, you will recover in the recovery area for approximately one hour, until you are alert enough to go home. There will be tapes over the incision which should remain intact for 2 weeks.
The Day of Surgery
While you're making preparations, be sure to arrange for someone to drive you home after your surgery and to help you out for a few days if needed. Your surgery will be performed in our outpatient surgery center at 11190 Warner Avenue, Suite 400; Fountain Valley, CA. You will not be allowed to have anything to eat after 9pm the evening before surgery or drink after midnight the night before surgery. You should shower prior to surgery but do not use any deodorant, lotions, or perfumed products on your arms. You should not bring jewelry, contact lenses, or valuables on the day of surgery. Wear loose, comfortable clothing with a shirt that buttons or zips in the front (we don't want to put anything on over your head when you get out of surgery). Arrive two hours prior to surgery.
After Surgery
For the first day or two you will rest most of the time and limit your upper body activity. You should sleep with your head elevated (approximately 45 degree angle) so that you can get in and out of bed without using your arms. You will begin taking pain medication and antibiotics after surgery. Each time you take your pain medication, you should have a snack as the medication can be irritating to your stomach. For the first 24 to 48 hours you will have a sensitive stomach and you should eat mild foods such as you would normally eat if you had the flu (soups, crackers, etc.). Dr. Nguyen also strongly suggests Gatorade to replenish your fluid levels.
You should plan to significantly reduce your activity for several days following your surgery. You will be limited to light arm movement for approximately one week after surgery. You should not reach or stretch your arms for two weeks. You will notice an improvement in the contour of your arms immediately. However, your shape will continue to improve in the following weeks as the swelling subsides. Your arms will be very sore for several days, but this will subside within two weeks. You will be able to return to most of your normal (light) activities within a couple of days. You will be able to shower once you feel comfortable being up for extended periods of time without lightheadedness, usually on the third day. The tapes over your incision will protect the incision from the soap and water so you may shower as normal and pat the tapes dry. Patients with an office job are often able to return to work within 7 days. More strenuous jobs may require work restrictions for eight weeks, which you may request a certifying letter from the receptionist.
You should not do any strenuous activities or heavy lifting (over 5 lbs) for 8 weeks after surgery. You will be followed closely for the first three months after surgery and will be advised which specific activities you may resume as your healing progresses. You should able to resume most of your normal daily activities within two weeks following surgery, and you should be able to resume all of your physical activities within eight weeks of surgery.
Once all incisions are well-healed (no scabs or open areas), you may begin applying ScarGuard to the incisions. This combination of hydrocortisone, silicone, and Vitamin E, is applied like nail polish and helps your scars fade as much as possible. ScarGuard is available at the pharmacy in our building.
Your Results
Your arms will be smaller and firmer. You will fit more comfortably into clothing and feel more confident in the way you look in short sleeves. Incisions may remain lumpy and red for months, then gradually become less obvious, flattening out and lightening in color.

Panniculectomy
Overview
The most common complaint of gastric bypass patients once they reach their goal weight is the loose hanging skin of the abdomen. The hanging skin of the abdomen, called the pannus or "apron" can cause a variety of problems for the patient. These include the inability to fit comfortably into clothing, difficulty in daily hygiene, yeast infections or cysts in the folds of skin, and lower back pain. The removal of this excess skin is acheived by panniculectomy, tummy tuck, or body lift procedures.
The term panniculectomy is used to describe the removal of the hanging, excess skin of the abdomen. This procedure does not include muscle tightening or liposuction. A panniculectomy may be covered by insurance if your plan allows for it and if you meet a series of standards set forth by the insurance regulators. Removal of excessive skin over the abdomen may be covered if the pannus (hanging skin) occludes the pubic area and is causing health problems. Documentation from your primary physician of back pain, rashes, yeast infection of the skin under the pannus, or other physical symptoms may be required. Also, documentation may be necessary to show that conservative measures have been attempted and have failed to alleviate symptoms. These measures may include chiropractor visits, pain medications, topical ointments for skin infections and rashes, etc. Photos will be taken at the time of consult to show the severity of the pannus and skin rashes/lesions if present.
A Panniculectomy is not a Tummy Tuck and does not include tightening of the muscle or liposuction. A tummy tuck, in contrast to a panniculectomy, connsists of three components: muscle tightening, liposculpture, and removal of excess skin. Your surgery will be performed as an outpatient surgery at the hospital covered by your insurance plan. You will not be allowed to have anything to eat or drink after midnight the night before surgery. You should shower prior to surgery but do not use any lotions or perfumed products on your body. You should not bring jewelry, contact lenses, or valuables on the day of surgery. Wear loose, comfortable clothing and arrive two hours prior to surgery.
After Surgery
For the few days you will rest most of the time. You will need help getting in and out of bed and getting to the restroom. You should rest with your upper body elevated (approximately 45 degree angle) so that you can get in and out of bed more easily. One or two pillows should be placed under your knees for comfort.
You will begin taking pain medication and antibiotics after surgery. Each time you take your pain medication, you should have a snack as the medication can be irritating to your stomach. For the first 24 to 48 hours you will have a sensitive stomach and you should eat mild foods such as you would normally eat if you had the flu (soups, crackers, etc.). Dr. Nguyen also strongly suggests Gatorade to replenish your fluid levels.
The drainage tubes are usually removed within the first week, during your follow-up appointment. If drainage continues to be more than 25cc in a 24 hour period, you may be rescheduled to return for drain removal. You may shower the day after the drains are removed. The tapes over your incision will protect the incision from the soap and water so you may shower as normal and pat the tapes dry. The surgical tape will begin to fall off during the second week and can be removed at the end of the second week. Patients with an office job are often able to return to work after about two weeks. More strenuous jobs may require work restrictions for eight weeks, which you may request a certifying letter from the receptionist.
Over several weeks, the swelling will continue to subside, bruising will fade, and normal sensation will return. Although you will notice a difference in the contour of the area shortly after surgery, the improvement will become even more apparent as the swelling and fluid retention subsides over several weeks to months. You will notice that your clothing fits more comfortably and you will enjoy a new, slimmer appearance. Most patients are very happy with their firmer, flatter abdomen and their renewed confidence in the way they look in clothes. Your new contours will be permanent providing your weight is stable. In addition, most patients notice that changes in weight do not result in accumulation in this area as it did before.
After two weeks, and once all incisions are well healed (no open areas or scabs), you can begin using Scar Guard® scar treatment or Neosporin Scar Solution® on the incisions. Scar Guard® and Neosporin Scar Solution® are available at the pharmacy in our building.

Thigh Lift
Overview
The combined effects of aging, gravity, weight loss, and reduced skin elasticity all contribute to loss of tone in the thighs. Loose thighs may reduce the clothing styles you are able to wear and may cause problems such as rubbing and rashes on the inner thighs. A thigh lift is a surgical procedure to remove excess skin and fat from the thighs resulting in improved contour of the legs. A thigh lift may be performed alone or in combination with other body contouring procedures.

Dermabrasion
Overview
Dermabrasion is a skin resurfacing technique used to smooth out imperfections in the skin. It is commonly performed to lessen the appearance of scaring from acne, accidents, or previous surgeries as well as reduce the appearance of fine lines, especially those around the mouth. If you desire to lessen the appearance of scarring or fine lines, dermabrasion may be a good choice for you.
Dermabrasion involves the use of a surgical instrument to selectively remove the top layers of skin that are affected by surface imperfections. The surgeon uses a burr containing diamond particles attached to a motorized handle which scraps the skins surface until the safest level has been reached that will lessen the look of the scar or wrinkle. The procedure may be performed on a small area or the entire face. Dermabrasion is usually performed under local anesthesia. However, in more severe cases you and your surgeon may elect for general anesthesia.

Preparing For Surgery
On the day of your consultation, you will receive instructions on how to optimize your health before surgery. This will include what medications to stop and what testing will be required before surgery. If you will be undergoing general anesthesia and are 45 years of age or older -or- have any health problems, you will need to see your primary care physician to have bloodwork, an EKG, and obtain surgical clearance stating that there are no counterindications for you having surgery. You should not take aspirin containing products or smoke for two weeks before surgery. If you have health problems or a history of health problems, you may require additional testing or clearance.
The Procedure
The procedure is normally performed as an outpatient surgery under local anesthesia and takes approximately 20-45 minutes. After the procedure, you will recover in the recovery area for a short time. After the procedure, an antibiotic ointment will be placed on the skin to keep it moist and help reduce your discomfort.
The Day of Surgery
While you're making preparations, be sure to arrange for someone to drive you home and be with you overnight. Your surgery will be performed in our outpatient surgery center at 11190 Warner Avenue, Suite 400; Fountain Valley, CA. If done alone, under local anesthesia, you may eat and drink as normal. If you are having the procedure under general anesthesia, you may not have solid foods after 9pm or water after midnight, the night before surgery. You should have clean, dry hair. Wear no makeup or moisturizers on your face. You should not bring jewelry, contact lenses, or valuables on the day of surgery. Wear loose, comfortable clothing with a shirt that buttons or zips in the front (we don't want to put anything on over your head when you get out of surgery). You will need someone to bring you and take you home on the day of your procedure. If you are having local anesthesia, arrive one hour prior to surgery. If you are having general anesthesia, arrive two hours prior to surgery.
After Surgery
Immediately, your face will be red and swollen and you will likely experience some tingling, burning, or aching sensations. You will be given pain medication to help control the amount of discomfort you feel. You may use ice packs wrapped in a clean cloth to soothe your skin.
You will keep a layer of ointment on your skin for two weeks, until it is healed. You should rinse your face with tepid water two times per day and reapply the ointment. Some people may develop irritation from the antibiotic ointment. If you develop severe itching or a rash, discontinue the antibiotic ointment and use pure petroleum jelly instead.
The swelling will begin to lessen in a few days to a week. You will likely be able to return to work in approximately two weeks. Your skin will continue to be slightly swollen and pink for several weeks as you begin to resume your normal activities. Strenuous activities should be avoided for six weeks. It is especially important to avoid the sun during the healing process while normal pigmentation returns, as long as six to twelve months. Several months following the procedure, your normal pigmentation will return and your skin will be much smoother than before.
Your Results
You can expect to enjoy a fresher, smoother appearance. Fine lines around the mouth can be minimized, acne scarring on the cheeks and forehead will be lessened. Although your skin will continue the normal aging process, you will do so beginning with your new fresh appearance. The results are long-lasting and you will enjoy your new refreshed look for years to come. The procedure may be repeated after 6 months if desired.

Chemical Peel
Overview
A chemical peel is a skin resurfacing procedure that reduces the appearance of blemishes, fine surface wrinkles, and uneven pigmentation by removing the damaged outermost layers of skin with a chemical solution. The procedure can be performed alone or in conjunction with other facial rejuvenation procedures such as eyelid surgery or facelift. Depending on your skin type or individual indications, your surgeon may require a pretreatment regimen including Retin-A, Hyrdoquinone, or a combination of the two.
In preparation for your procedure, you will need to arrange for someone to bring you to your appointment and take you home. During the procedure, a chemical solution (usually containing trichloroacetic acid) is applied to the skin for approximately 15 minutes. The peel will be adjusted in strength, allowing a superficial to deep peel, depending on your needs. The solution has anesthetic properties and therefore no anesthesia is necessary. After the procedure, an ointment will be applied to your skin to help keep it moist and protected. Depending on the depth of the peel, you will likely experience moderate discomfort and mild swelling that will subside within the first week. You will rinse your face twice a day with tepid water and reapply the ointment. If you develop a sensitivity to the ointment (itching or rash) switch to pure petroleum jelly. In approximately 7-10 days, your skin will have healed enough for you to resume many of your normal activities. Until your skin has healed, it is important to avoid sun exposure. For several months, you will need to continue to avoid sun exposure unless your skin is adequately protected using a sunscreen (minimum SPF 30). Your final result will be skin that is smoother, tighter, and healthier. A chemical peel may be repeated after several months if additional correction is desired.
