Breast Augmentation

Am I a good candidate for breast augmentation?

Breast augmentation, also known as augmentation mammaplasty, is a procedure that enhances the size and shape of a woman's breast. In 2005, more than 350,000 breast augmentation surgeries were performed in the United States. It is one of the more common procedures performed by Board Certified plastic surgeon Dr. Tyrone.

Generally, most healthy women are candidates for breast augmentation surgery. Age is not a contraindication to breast implant surgery, and it is not uncommon for women in their 50's or later to undergo the procedure. Breast augmentation can enhance your appearance and your self-confidence, but it won't necessarily change your looks to match your ideal, or cause other people to treat you differently. Before you decide to have surgery, think carefully about your expectations and discuss them with Dr. Tyrone. If you're physically healthy and realistic in your expectations, you may be a good candidate.

What types of implants are available for breast augmentation surgery?

A breast implant is a medical device with a silicone shell filled with either silicone gel or a salt-water solution known as saline. Most cosmetic breast augmentation involves the placement of saline-filled implants behind the breast or chest muscle. For the majority of women, saline implants are safe and reliable, and make and excellent choice. For a select group of patients, Dr. Tyrone may recommend silicone gel breast implants. These include patients with very thin soft tissues coverage of the chest, those who require a breast lift in conjunction with an augmentation and those patients who require implants following breast cancer surgery. Silicone gel implants are available to women who meet inclusion criteria determined by the FDA. This includes most women over the age of 22 years old.

Dr. Tyrone will discuss with you the advantages and disadvantages of each type of breast implant, and help you make a decision that is best for you.

What will my breast augmentation consultation with Dr. Tyrone entail?

During your initial breast augmentation consultation, Dr. Tyrone will inquire about your medical and surgical history including current medications and allergies. It is important to be thorough when discussing your medical history as safety is Dr. Tyrone's top priority. Your full disclosure of all pertinent medical information prior to surgery will reduce the incidence of unwanted surgical omplications. You will discuss your concerns and motivations for seeking breast augmentation. Details of the breast augmentation will be discussed including incision placement, implant type and size as well what is expected of you preoperatively and postoperatively. The risks of surgery will also be discussed in detail. Numerous before and after photographs of actual patients will be reviewed during your consultation. Dr. Tyrone will be able to tell you where the incisions in the photographs are located as well as the size of the implants. Frequently, during this part of the breast augmentation consultation, the patient sees a photograph that she feels is most similar to the before and after result she would like to obtain.

Following your consultation, you will meet with our patient coordinator, to discuss scheduling your surgery and answer any other questions you may have.

What do I need to do to prepare for my breast augmentation surgery?

Once you have scheduled your breast augmentation surgery with Dr. Tyrone, you will be given preoperative instructions that discuss everything from finances to follow-up visits after your surgery. Prior to your breast augmentation, you will receive prescriptions form Dr. Tyrone's patient coordinator. It is important to have the medication prescriptions filled in the days prior to your surgery so they are waiting for you when you return home from surgery.

You will receive a list of medications (i.e. aspirin and nonsteroidal anti-inflammatory drugs) to avoid for 14 days prior to your breast augmentation surgery. Additionally, though there is no scientific proof that certain medications reduce bruising, swelling and pain after breast augmentation surgery, you will be asked to obtain Arnica Montana, Bromelain and Vitamin A pills to take prior to and after your procedure. Finally, the night before surgery, shower and wash as you would normally and do not eat or drink after midnight.

Where will my surgery take place?

Dr. Tyrone performs breast augmentation surgery in a new, state of the art operating facility located in Gainesville, Florida. The Laser and Outpatient Surgery Center, located in Gainesville, Florida is certified at the highest level by American Association for Accreditation of Ambulatory Surgery Facilities. What results can I expect?

Provided you are a good candidate for breast augmentation surgery, breast implants will provide you with the increased size and fullness you are seeking. The results will be long lasting. It is important to remember that your breast will continue to age on top of the implant. Breast shape may also subsequently change secondary to pregnancy and fluctuations in weight. Problems with the implants themselves are rare; however, implant rupture may occur and require the placement of a new silicone or saline breast implant. Maintaining a relationship with Dr. Tyrone after your breast augmentation surgery is important to help you address these changes and other concerns which may arise. Regular examination by Dr. Tyrone and routine mammograms for those in the appropriate age groups at prescribed intervals will help assure that any complications, if they occur, can be detected early and treated.

What are the risks of breast implant surgery?

Breast augmentation is relatively straightforward procedure, and most patients have very low risks of complications. But as with any operation, there are risks associated with surgery and specific complications associated with this procedure.

The most common problem, capsular contracture, occurs if the scar or capsule around the implant begins to tighten. This squeezing of the soft implant can cause the breast to feel hard. Capsular contracture can be treated in several ways, and sometimes requires either removal or "scoring" of the scar tissue, or perhaps removal or replacement of the implant. Massage of the breast implants after surgery has been demonstrated to decrease the incidence of capsular contracture. Dr. Tyrone will instruct you on the proper massage technique following surgery, and it is important to follow his recommended regimen.

As with any surgical procedure, excessive bleeding following the operation may cause some swelling and pain. If excessive bleeding continues, another operation may be needed to control the bleeding and remove the accumulated blood.

A small percentage of women develop an infection around an implant. This may occur at any time, but is most often seen within a week after surgery. In some cases, the implant may need to be removed for several months until the infection clears. A new implant can then be inserted.

Some women report that their nipples become oversensitive, undersensitive, or even numb. You may also notice small patches of numbness near your incisions. These symptoms usually disappear within time, but may be permanent in some patients.

There is no evidence that breast implants will affect fertility, pregnancy, or your ability to nurse. If, however, you have nursed a baby within the year before augmentation, you may produce milk for a few days after surgery. This may cause some discomfort, but can be treated with medication prescribed by your doctor.

Occasionally, breast implants may break or leak. Rupture can occur as a result of injury or even from the normal compression and movement of your breast and implant, causing the man-made shell to leak. If a saline-filled implant breaks, the implant will deflate in a few hours and the salt water will be harmlessly absorbed by the body.

If a break occurs in a gel-filled implant, however, one of two things may occur. If the shell breaks but the scar capsule around the implant does not, you may not detect any change. If the scar also breaks or tears, especially following extreme pressure, silicone gel may move into surrounding tissue. The gel may collect in the breast and cause a new scar to form around it, or it may migrate to another area of the body. There may be a change in the shape or firmness of the breast. Both types of breaks may require a second operation and replacement of the leaking implant. In some cases, it may not be possible to remove all of the silicone gel in the breast tissue if a rupture should occur.

While there is no evidence that breast implants cause breast cancer, they may change the way mammography is done to detect cancer. When you request a routine mammogram, be sure to go to a radiology center where technicians are experienced in the special techniques required to get a reliable x-ray of a breast with an implant. Additional views will be required. Ultrasound examinations may be of benefit in some women with implants to detect breast lumps or to evaluate the implant.

While the majority of women do not experience these complications, Dr. Tyrone will discuss each with you to make sure you understand the risks and consequences of breast augmentation.

Breast Reduction

Am I a good candidate for breast reduction surgery?

Breast reduction surgery, also known as reduction mammaplasty, is a procedure that reduces the size and enhances the shape of a woman's breast. It is one of the more common procedures performed by Board Certified Plastic Surgeon Dr. Tyrone. Good candidates for breast reduction surgery are patients who have large pendulous breasts, large breasts which are out of proportion to the rest of their body and patients who have one breast which is much larger than the other (ie breast asymmetry). Patients with large breasts often have neck and back pain as well as skin irritation in between and underneath their breasts. Patients with macromastia (ie large breasts) also frequently have bra-strap grooving. If you have large breasts and symptoms as previously described, insurance may cover the costs of your surgery.

Dr. Tyrone performs breast reduction surgery with the latest, most refined techniques available. Following a breast reduction, symptoms of back and neck pain as well as skin irritation are often corrected. Though scarring is permanent, the scars do fade over time. Incisions are typically located around the areola (pigmented skin around the nipple) extending down to the fold underneath the breast, and underneath the breast itself. There are many "short scar" techniques that eliminate the incision placed underneath the breast. Dr. Tyrone will let you know during your consultation if you are a candidate for a minimal incision breast reduction. Candidates for a minimal incision breast reduction are frequently those patients seeking a small reduction and have minimal excess breast skin and stretch marks. When a short scar breast reduction technique is used, liposuction is frequently utilized to reduce the lateral and axillary breast fullness. For patients seeking a breast reduction who do not wish to have any scars, a lipo-breast reduction may also be an option (utilizing liposuction to reduce the volume of the breasts with several small incisions). This may be an option if skin quality of the breast is good and minimal nipple elevation is required. Just as any other surgical scar or incision, incisions from breast reduction surgery predictably mature and fade over time. Dr. Tyrone will instruct you on how to optimize and support your incisions in the postoperative period to obtain the best scar possible.

Breast Lift

Am I a good candidate for breast lift surgery?

Breast lift surgery, also known as mastopexy, is a procedure that enhances the shape of a woman's breast. Breast shape is affected by numerous factors including loss of skin elasticity that occurs with aging, pregnancy, breast feeding and fluctuation in a woman's weight. Breast Reconstruction

Am I a good candidate for breast reconstruction?

If you have been diagnosed with breast cancer and are having or had a mastectomy, you owe yourself a consultation with a plastic surgeon to discuss breast reconstruction. Most patients are candidates for breast reconstruction. Breast reconstruction is generally performed after mastectomy for breast cancer, but may also be performed to correct a congenital absence or acquired loss of the entire or a portion of the breast. . The procedure may be either "immediate" or "delayed", and may involve the use of an implant or transfer of your own tissues to reconstruct the breast. Often times, a combination of your own tissue with an implant is used. "Immediate reconstruction" is usually performed during the same operation as the mastectomy, while "delayed reconstruction" is performed at some later date after the tissues have healed. Reconstruction with an implant may involve placement of a tissue expander to create a pocket for either a saline or silicone gel filled implant. As with any implant, additional surgical procedures may be necessary in the future to correct changes or replace the implant. In contrast, autologous reconstruction involves the transfer of your natural tissues onto the chest to reconstruct the breast mound. The most commonly utilized techniques are the TRAM (transverse rectus abdominus myocutaneous) and LD (latissimus dorsi) flaps. These flaps typically require a longer initial surgical procedure and a short in-patient hospitalization, but can give excellent long term results. Breast reconstruction is not a simple procedure. There are often many options to consider as you and your doctor explore what's best for you. Dr. Tyrone can best educate you on which procedure may be best for you.

What are the basic types of breast reconstruction performed by Dr. Tyrone?

The goal of breast reconstruction is to create a breast mound that matches the remaining normal breast in size and shape. Dr. Tyrone utilizes numerous techniques to achieve this goal. Any breast reconstruction involves the use of an implant or a "flap". Some techniques of breast reconstruction utilize an implant and a "flap". "Flaps" used in breast reconstruction are composed of muscle, fat and skin which are transferred from the abdomen or back to the breast. Once the "flap" is transferred to the breast, it is shaped to reconstruct the breast mound. The most common flaps used by Dr. Tyrone are the transverse rectus abdominus myocutaneous flap (TRAM flap) and the latissimus dorsi myocutnaeous flap. The benefit of flap reconstruction of the breast is that a breast mound can often be created without the use of an implant and frequently the entire reconstruction only requires one operation under general anesthesia). Breast reconstruction using expanders and implants is often the easiest in terms of recovery for the patients. An expandable implant is placed at the time of your mastectomy in cases of immediate breast reconstruction; this expandable implant is replaced with a saline or silicone-filled breast implant after it has been expanded to a volume that matches the unaffected breast. In cases of bilateral reconstruction, the expanders are expanded symmetrically. Using expanders and implant alone for breast reconstruction requires at least 2 surgical procedures. Furthermore, in thinner patients, the implants are frequently palpable and implant rippling may be noticeable. Problems with rippling and palpability have been reduced by using Alloderm® for inferior implant coverage. Alloderm is placed at the time of the mastectomy and helps to better define the fold underneath and lateral to the breast and provides additional padding where muscle coverage of the implant is more difficult ("Alloderm Sling").

What will my breast reconstruction consultation with Dr. Tyrone entail?

During your initial breast reconstruction consultation, Dr. Tyrone will inquire about your medical and surgical history including current medications and allergies. It is important to be thorough when discussing your medical history as safety is Dr. Tyrone's top priority. Your full disclosure of all pertinent medical information prior to surgery will reduce the incidence of unwanted complications after surgery. Dr. Tyrone will also contact your surgical oncologist or breast surgeon prior to your consultation so he will be aware of what your cancer surgery will entail (removal of a portion or your entire breast) and whether or not it seems likely that you will need postoperative radiation therapy. Both of these issues affect your breast reconstruction. Your objectives and goals will be discussed. Dr. Tyrone will then discuss the different types of reconstruction available. Photographs of patients reconstructed with a variety of techniques will be shown to you.

You will then be asked to put a gown on. Photographs will be taken. Dr. Tyrone will then review your photographs with you. Differences between the 2 breasts (i.e. asymmetry) will be noted and proposed incisions demonstrated. Computer imaging is an invaluable part of the consultation. Dr. Tyrone encourages you to ask any questions during you consultation. Following your consultation, you will meet with Dr. Tyrone's patient coordinator, to discuss scheduling your surgery and answer any questions you may have forgotten to ask Dr. Tyrone. Frequently, Dr. Tyrone recommends surgery on the normal breast that is not affected by cancer. He will do this if the normal breast is particularly large or droopy as it is difficult for a reconstructed breast to attain the same degree of size and droop that is frequently present.

Liposuction

Am I a good candidate for liposuction surgery?

Liposuction is the ideal treatment for patients at or slightly above their normal weight who are bothered by isolated pockets of fat that significantly take away from their body's contour. Liposuction is not a treatment for obesity and it is not recommended in patients who have a significant amont of excess skin or skin of poor quality (significant stretch marks). For patients with excess skin and poor quality skin, excisional surgery such as abdominoplasty or tummy tuck surgery is a better option. Liposuciton works by reducing the volume or amount of fat in a particular area and then allowing the skin to redrape, contract and shrink to envelope the reduced volume. Skin with stretch marks and skin of older pateitns may not tighten over the area which has been reduced by liposuction. Some of the more common areas treated by liposuction are the abdomen, hips, flans, lateral thighs, medical thighs, medial knees, chin and arms.

What is liposuction, suction lipectomy or lipoplasty?

In liposuction, fat is removed via a small, hollow tube called a cannula. Incisions are placed in areas where the body's natural creases will minimize any potential scarring. Incisions are smaller than a centimeter in length. Once the incision is made, solution containing anesthesia and epinephrine ("wetting solution") are infused to minimize pain and blood loss. Once the area to be treated has been infused with wetting solution, the cannula is connected to a suction unit and guided by the expert hands of Dr. Tyrone. The cannula device then literally suctions away the unwanted fat - permanently. Care is taken to avoid irregularities in the skin caused by aggressive superficial liposuction. Care is also taken to avoid causing depressions from overly aggressive fat removal.

Am I a good candidate for a tummy tuck?

Abdominoplasty, also known as a "tummy tuck", is a procedure which removes excess skin and fat from the abdomen. The abdominal and waist line contours are improved by removing the excess skin and fat, reuniting the abdominal muscles in the midline and contouring the remaining fat by a combination of direct excision and liposuction. If you have a significant amount of excess fat, a secondary liposuction procedure may be required as overly aggressive liposuction at the time of abdominoplasty may compromise wound healing. Where will my abdominoplasty scars be?

Dr. Tyrone will be able to give you an excellent idea of where your abdominoplasty scars will be prior to your operation. They will be shown to you during your examination. You will also be marked prior to your surgery. You will be asked to wear a bathing suit or underwear at the time of your preoperative markings so the incisions can be designed to fall in the confines of your undergarments. The scar typically lies above the pubic hair and extends a variable distance to the sides below the hip bones.

During a full abdominoplasty, as opposed to a mini-abdominoplasty, the belly button is left attached to the abdominal wall and brought out through a new opening after the excess skin has been removed. The location of your belly button does not change. It is impossible to know exactly where your scar will lie and to know the exact length of your scar prior to your operation. Scars will be discussed in detail prior to your operation. This is because the final adjustments to the scar including location and length are made during the operation. Every effort will be made to leave you with as fine a scar as possible by suturing not only the skin at the time of your operation but also repairing the deeper fascia (superficial fascial system) of the abdominal wall. By repairing the deeper layers of the abdominal wall, tension on the skin repair is minimized. Scars will be discussed in detail prior to your operation. Dr. Tyrone will also instruct you on how to optimize the appearance of your scars after surgery. Dr. Tyrone will make every effort to leave a scar that can be concealed in your undergarments. It is important to remember that scars often get worse before they get better. Most scars undergo a period of time where they are swollen and red typically 4-8 weeks after surgery before they begin to settle.

What do I need to do after my abdominoplasty?

After recovering from your tummy tuck surgery with Dr. Tyrone, you will be able to go home in the company of a friend or family member. If you have chosen to have your surgery in a hospital setting (or Dr. Tyrone felt it was medically indicated to you're your surgery in the hospital), you will be discharged the day after your surgery. You will receive thorough postoperative instructions. Dr. Tyrone will call you at home or at the hospital the night of your abdominoplasty. Your pain will be well controlled and you will be able to drink normally. Your incision will be covered by steristrips that are not to be removed, a gauze dressing, and an abdominal binder. For the first few days after surgery the binder should not be made too tight. If you had a full abdominoplasty, you will typically have two drainage tubes. You will be instructed in their care. Usually, one tube is removed @ 3-5 days and the remaining tube will be removed a 710 days. You will be permitted to shower after both drains have been removed. Compression stockings as recommended in your preoperative instructions should be worn until you are ambulating normally and with regularity. It is important to be mobile after surgery and to keep blood circulating in your legs. You may have difficulty standing straight up after surgery; however, this will pass after several days.

You may shower on the second day after surgery. Drains are typically removed 7-10 days after the operation, Your incisions will be supported by steristrips or surgical tape for 4-6 weeks. As discussed, scars tend to get worse initially and then fade over time. Dr. Tyrone will discuss how to care for your incisions. You may resume aerobic activities after three weeks but should refrain from abdominal exercises or heavy lifting for six weeks.

Dr. Tyrone is available at all times through his Gainesville office should a problem arise. Abdominoplasty and tummy tuck surgery patients are typically seen the day after their surgery, and one week later.

Gynecomastia

Under certain circumstances, a man can develop fibro-fatty breast tissue that resembles a woman's breast. This condition is referred to as gynecomastia. The etiology of this process is not widely understood, however, it is known that certain medical conditions, medications, hormones, drugs, and other factors may play a role. In the majority of cases, no etiology can be determined. Gynecomastia affects an estimated 40 to 60 percent of men, and it may affect one breast or both.

Depending on the extent of the problem, gynecomastia may be treated with excision of the breast bud, liposuction of the fatty tissue, or a combination of both procedures. Occasionally, a secondary procedure to reduce excess skin or to return the nipple to a more normal position may be needed. Results can vary, but generally patients have a chest that is flatter, firmer, and better contoured, and they often feel more comfortable in form fitting clothing or going without a shirt.

What causes gynecomastia?

Gynecomastia can be caused by certain medications, drugs, hormones and metabolic disorders, but most often it's cause is unknown and not due to any specific medical condition.

How is gynecomastia treated?

If gynecomastia is due to a drug or medication reaction, it can often be improved by stopping the offending agent. However, the majority of cases will require surgery to correct the problem. Dr. Tyrone typically uses a combination of liposuction and excision of breast tissue to correct the problem. Rarely a second operation may be required to correct skin laxity if the gynecomastia is severe.

Am I a candidate for gynecomastia surgery?
